Neonatal Resuscitation Program (NRP) 8th Edition Provider Curriculum (Essentials and Advanced)

Neonatal Resuscitation Program (NRP):

The Neonatal Resuscitation Program® (NRP®) course conveys an evidence-based approach to care of the newborn at birth and facilitates effective team-based care for healthcare professionals who care for newborns at the time of delivery. NRP utilizes a blended learning approach, which includes online testing, online case-based simulations, and hands-on case-based simulation/debriefing that focus on critical leadership, communication, and team-work skills.

The NRP Provider Course introduces the concepts and basic skills of neonatal resuscitation. It is designed for health care professionals involved in any aspect of neonatal resuscitation, including physicians, nurses, advanced practice nurses, nurse midwives, licensed midwives, respiratory care practitioners, and other health care professionals who provide direct care during neonatal resuscitation.

Steps to achieving provider status are listed below for both 7th and 8th edition. If you are taking 7th edition, you can review the  Provider LMS How-To Guide  to help you get started in the new NRP LMS and enroll in the NRP Provider Curriculum.

NRP 8th Edition Provider Curriculum (Essentials and Advanced)

This section describes the Instructor-led (traditional) format NRP course. Another option for 8th edition NRP is RQI® for NRP®, which is a quality improvement program for institutions. The program allows learners to complete NRP Essentials via a self-directed curriculum with quarterly activities to reinforce and master key NRP knowledge and NRP Essentials skills at a simulation station.  RQI for NRP is not required for 8th edition NRP training for those taking the Instructor-led course option.

Essentials or Advanced

For the 8th edition of NRP there are two different types of Provider: Essentials and Advanced.  NRP Essentials covers lessons 1 through 4 of the Textbook of Neonatal Resuscitation, 8th Edition. Anyone who is involved in the care of a newborn should take NRP Essentials. NRP Advanced covers lessons 1 through 11 of the textbook.  It is appropriate for those who attend births and are responsible for anticipated resuscitation of a newborn with known risk factors, and for those who participate in neonatal resuscitation beyond positive-pressure ventilation.

Each organization will determine its own policy for who should attain NRP Essentials or NRP Advanced provider status. If a healthcare professional is unsure about which NRP level to take, they should probably take NRP Advanced and should check with their institution, if applicable.

Anyone wishing to become an NRP Instructor Candidate must have a current NRP 7th edition Provider card or a current NRP 8th Edition Advanced provider card.

Provider Curriculum, Part 1

Part 1 focuses on building a foundation of neonatal resuscitation knowledge. The learning activities in Part I include:

  • Self-study the Textbook of Neonatal Resuscitation, 8th Ed
  • Online Learning Assessment
    • Featuring True Adaptive™ learning – a personalized method tailored to individual knowledge and confidence levels. True Adaptive learning adjusts to the learner’s competence and performance in real-time.
  • Advanced Exam (35 questions; for Advanced Curriculum only)
    • You have unlimited attempts to complete the exam
  • Part 1 Evaluation

Provider Curriculum, Part 2

Part 2 includes the Instructor-led Event (hands-on portion of the course). You will need to attend an Instructor-led Event appropriate to your curriculum type (Essentials or Advanced; some events may include both).  Activities include:

  • Register for an Instructor-led Event, which includes:
    • Performance and Integrated Skills
      • The Performance Skills Station provides an opportunity to practice or review technical hands-on resuscitation skills with instructor assistance.
    • The Integrated Skills Station allows the instructor to facilitate more than one scenario and evaluate the learner’s readiness for simulation and debriefing.
    • Simulation and Debriefing
      • Simulation and debriefing provide a safe setting in which to integrate cognitive and technical skills and focus on team communication and patient safety. The instructor will not coach, assist, or interrupt during a scenario.

You must register for an NRP 8th edition Instructor-led Event before attending the event. You can either register yourself for events or you can be registered by an instructor or administrator.

Once you complete all steps, the instructor will approve your skills and your eCard will be generated.  You will then have access to your NRP Provider eCard in your account on the NRP 8th edition Learning Platform.

How Palm Desert Resuscitation Education LLC (PDRE) Integrates NRP 8th Edition Practice Changes into NRP 7th Edition Provider Courses

Overview of Significant NRP 8th Edition Practice Changes (Rev. 5/21)

  • PRACTICE CHANGE:
    • Umbilical cord management plan added to 4 pre-birth questions, replacing “How many babies?”
      • NRP 7th Edition
        • The 4 pre-birth questions: (1) Gestational age? (2) Amniotic fluid clear? (3) How many babies? (4) Additional risk factors?
      • NRP 8th Edition
        • The 4 pre-birth questions: (1) Gestational age? (2) Amniotic fluid clear? (3) Additional risk factors? (4) Umbilical cord management plan?
  • PRACTICE CHANGE:
    • Initial steps reordered to better reflect common practice.
      • NRP 7th Edition
        • Initial steps: Warm and maintain normal temperature, position airway, clear secretions if needed, dry, stimulate.
      • NRP 8th Edition
        • Initial steps: Warm, dry, stimulate, position airway, suction if needed.
  • PRACTICE CHANGE:
    • An electronic cardiac monitor is recommended earlier in the algorithm
      • NRP 7th Edition
        • An electronic cardiac monitor is the preferred method for assessing heart rate during cardiac compressions.
      • NRP 8th Edition
        • When an alternative airway becomes necessary, a cardiac monitor is recommended for the most accurate assessment of the baby’s heart rate.
  • PRACTICE CHANGE:
    • Epinephrine intravenous/intraosseous (IV/IO) flush volume increased.
      • NRP 7th Edition
        • Flush IV/IO epinephrine with 0.5 to 1 mL normal saline
      • NRP 8th Edition
        • Flush IV/IO epinephrine with 3 mL normal saline (applies to all weights and gestational ages)
  • PRACTICE CHANGE:
    • Epinephrine IV/IO and endotracheal doses have been simplified for educational efficiency. The dosage range is unchanged. The simplified doses (IV/IO and ET) do not represent an endorsement of any particular dose within the recommended dosing range. Additional research is needed.
      • NRP 7th Edition
        • Range for IV or IO dose = 0.01 – 0.03 mg/kg (equal to 0.1 – 0.3 mL/kg)
        • Range for endotracheal dose = 0.05 – 0.1 mg/kg (equal to 0.5 – 1 mL/kg)
      • NRP 8th Edition
        • The suggested initial IV or IO dose = 0.02 mg/kg (equal to 0.2 mL/kg)
        • The suggested endotracheal dose (while establishing vascular access) = 0.1 mg/kg (equal to 1 mL/kg)
  • PRACTICE CHANGE:
    • Expanded timeframe for cessation of resuscitative efforts
      • NRP 7th Edition
        • If there is a confirmed absence of heart rate after 10 minutes of resuscitation, it is reasonable to stop resuscitative efforts; however, the decision to continue or discontinue should be individualized.
      • NRP 8th Edition
        • If confirmed absence of HR after all appropriate steps performed, consider cessation of resuscitation efforts around 20 minutes after birth (decision individualized on patient and contextual factors).

The NRP Online Examination:

  • The online exam is required before participants attend the classroom portion of the NRP course.
  • The hands-on course must be attended within 90 days of completing the online examination.
  • The instructor can see who has completed the online examination by looking at the class roster prior to the course.
  • The fee for the online examination can be managed in several different ways.

eSim – An Online Simulation Exercise (7th Edition NRP course):

  • eSim is required before the classroom portion of the NRP course and allows learners to practice textbook knowledge and decision-making skills in a virtual 3-D environment.
  • Because learners arrive at the in-person course having already practiced NRP steps in sequence, more time is available during the course to improve hands-on skills and work on teamwork and communication.

The In-person Portion of the Provider Course at PDRE Includes 3 Components:

  1. Performance Skill Stations, PSS (optional)
    • PSS provide an opportunity to practice or review technical hands-on skills with instructor assistance.
    • The Instructor determines which PSS need to be included in their course based on their assessment of learners’ needs.
    • Even experienced learners benefit from this skills review and practice.
  2. The Integrated Skills Station, ISS (required)
    • The ISS evaluates each learner’s ability to demonstrate the steps in the NRP Flow Diagram in proper sequence, using proper technique, and without assistance from the instructor.
  3. Simulation and Debriefing (required)
    • This provides a safe setting in which to integrate cognitive and technical skills and focus on team communication and safety.
    • Debriefing is where the learning occurs, because learners reflect on the experience and critically evaluate their actions.
    • Simulation training depends on good methodology, not technology, such as expensive simulators.

What is the Length for PDRE’s NRP Provider Course?

  • NRP allows a great degree of flexibility in planning the course agenda.
  • The length of the course is dependent on the number of learning objectives and the skill level of learners.
  • The curriculum you are about to se typically takes 3 to 4 hours, depending on the experience and learning objectives of participants.

Continuing Education (CE) Credits:

  • Learners are offered continuing education credits for passing the online examination and eSim cases.
  • Credit is provided for physicians, nurses, respiratory care practitioners, emergency medical service personnel and other medical professionals.

Certification:

  • Students who successfully complete the NRP Course will receive a NRP Provider Course Completion Card, valid for two years:
  • Complete the online component for NRP
  • Actively participate in, practice and complete all learning stations in an in classroom-based course by an AAP NRP Instructor
