Membership Application Terms & Agreement

Membership Details:

Billed monthly; Premium and Executive memberships offer members the choice of Tier 1-3 or up to $260 credit per IV session.

Application and Agreement Terms:

Our membership wellness includes several benefits. As part of your membership wellness, your monthly treatment needs to be redeemed in the month you are paying dues. (within 30 days after you are charged your membership dues) Membership treatments that are not used in the month will expire. Advanced appointments made must be canceled 24 hours in advance or the appointment is considered a used treatment. Membership and benefits are non-transferable and non-refundable. Applicants for memberships must be at least 18 years old.

Automatic Payment Agreement: Your credit card will be kept on file by Replenish 360 and each month your credit card will be charged the amount recorded on the same date each month, “monthly dues date”. Your credit card and email address is required for participation in our membership wellness program. Replenish 360 reserves the right to review subscriptions periodically. Members will be given at least 30 days’ notice in writing of any changes, which include: (i) any increase in membership fee, (ii) change in date of automatic withdrawal. Surcharge and inventory fee may apply (15% for In-Office or 25% for Mobile Service plus Concierge fee).

Termination of Membership: This agreement may be terminated at any time by Replenish 360 or a member provided that at least thirty days notice is given in advance of your monthly dues date. Any unused treatments will expire. Memberships are a minimum of three months (Initial Period). A member shall not be entitled to terminate or suspend his/her membership during the Initial Period. After the Initial Period, the membership shall continue month to month. Membership cancellations must be a 30-day advance written notification. If any of the supplied information by the member is false or misleading, membership may be terminated.

Mandatory Session Prerequisites: A comprehensive medical and clinical history is obtained, which includes acquiring possible history of present illness, known drug allergies, prescription and non-prescription medication use, past medical history, family history, social history, and other relevant medical and clinical histories that are contraindicated with continuing any of our wellness services. Members must sign an informed consent and liability waiver, “Replenish 360 Intravenous Therapy Consent & Client Agreement/Client Representations, Warranties, & Disclaimer Agreement,” that fully states terms and conditions in addition to understanding the possible benefits, consequences, and risks of our treatments. You must advice us immediately if your medical circumstances change. Before seeking membership with Replenish 360 we ask that you see your primary care physician, specialist, and/or licensed medical provider for any chronic and acute medical illness(es) requiring clinical and comprehensive evaluations by a qualified healthcare provider before you proceed with our membership services.

We would need a pre-booking non-refundable deposit for our services to officially schedule our IV Hydration Therapy sessions. All appointments require a booking deposit for in-office and off-site services.

Cancellation and Rescheduling

  • More Than 48 Hours
    • Registration cancellations more than 48 hours from start of a service/appointment will receive a 100% refund, minus a $25 administrative fee per scheduled guests, clients, and customers.
    • Cancellations must be made within 48 hours of appointment time to receive a refund.
  • 48 Hours or Less
    • No refunds will be issued for cancellations 48 hours or less prior to the service/appointment. However, such guests, clients, and customers may attend a future service/appointment by paying a $35 rescheduling fee.
    • Once the service/appointment has begun or been rescheduled, no refunds will be given unless otherwise stated or agreed upon.
  • Lack of Participation/Emergency Situations
    • We reserve the right to cancel or reschedule a service/appointment due to the lack of participation or personnel/workplace emergency situations. Please be advised that notification of canceled or rescheduled service/appointment (s) will be given by phone or email as soon as possible.

This place is amazing. My brother in law was badly dehydrated and wanted to call an ambulance. We found this place and tried it first. We saved $500 on a hospital trip. I tried it also because I was curious and had a hangover. I tried the saline with vitamins (I think Myers blend) and left feeling great. I’ll be back during festival season and will def do it again. I’d like to try it when not hungover to see if there is still a boost because the difference was huge.

I’m convinced hiring Replenish 360 is amongst the best decisions I’ve ever made. I called the morning of my wedding to mitigate a moderate to advanced hangover. Deep head and neck aches, dehydration, body aches. We were on a sustained good mezcal / mediocre beer program the night prior. Two wonderful nurses came to me poolside whilst enjoying huevos rancheros and having our friends trespass to the private pool of our adorable little hotel. They told me the scoop, plugged me in, and we began the 45min road to recovery. There was no pain. You start feeling better about 20 mins in. The full flight takes about 4 hours to get to cruising altitude where you will find yourself 100% replenished. The vitamin, pain reliever, and saline cocktail is delicious. This is a steep ticket show, no doubt. But look, if you’ve got the big day / meeting / event etc… this is an investment in yourself. Very big returns. I’m just thrilled that the nurses and service we received was all so easy, helpful, and fun. 5 stars all the way.

Replenish 360 was my first IV experience apart from the hospital. Besides a good cheeseburger and hair on the dog, I heard that IV therapy was a great, clean way to cure yourself of a hangover. I was in town for a wedding, had a little too much and desperately needed relief, having a lot to do the next day. I found Replenish 360 through a simple Google search. I gave them a call, came into their facility on a weekend, and they treated me with a so called “banana bag” and was given some headache medicine. An hour or two later, it was like the wedding and all the drinking never happened. If you are in the area and in need of this relief, I highly recommend giving them a call and paying a visit.

A GREAT EXPERIENCE!! Let me just say that I hate being poked. I’ve always been pretty squeamish when it came to needles. I was a little hesitant but have been wanting to get IV Hydration for a while to see what the hype was about. I called them up and let them know I was in town with my husband and a few friends who wanted to get IV therapy. They said they were able to come to us and have it done in our hotel room! So awesome. We scheduled for the next morning. The staff was so friendly and very professional. They are all certified/licensed nurses and medical professionals so they know what they’re doing. My husband and I both went with Tier 3. We were able to get hydrated while we relaxed! It was an overall positive experience and I would definitely do it again!
